Physical Examination
All athletes are encouraged to have a physical examination each year. You can upload a digital copy of the physical form during registration or you can send in a hard copy to the Athletic Director's office.
Athletes at Waterville Senior High School are required to have a complete physical examination from their health care provider every 2 years to be eligible for athletic participation. The first physical examination must occur six months prior to entry at Waterville Senior High School.
Athletes at Waterville Junior High School are required to have a complete physical examination from their health care provider every 2 years to be eligible for athletic participation. The first physical examination should occur six months or sooner prior to participation on an athletic team.
Additional physical examinations may be mandatory for both WSHS and WJHS athletes in the event of a chronic medical condition or a recent athletic or non-athletic injury. Written documentation must be presented to the Athletic Director. Requirements for additional physical examinations are at the discretion of the Athletic Director, in conjunction with the Certified Athletic Trainer and/or School Nurse.
